Ok Plus application form
Application form available:

• on board planes
• in Czech Airlines sales offices
• at web site www.okplus.cz or www.ok-plus.com
• at IATA travel agencies

Each private individual aged 2 years and older may become a member of the OK Plus programme, if he/she complies all membership conditions.
membership conditions are possible to find in application form or at www.ok-plus.com or www.okplus.cz
New members joining the program from 1 April 2003 may add their flts /Czech Airlines flts only/ flown within 3 months
before the date of joining /required documents are copy of the air ticket and originals of boarding passes/.

Ok Plus cards

• *temporary card*
  Temporary card is a part of application form. The passenger detach the temporary card and fill in his/her name. A temporary card may also be printed on-line, through the application for on the internet.
• *identification card*
  OK Plus members will obtain an OK Plus identification card crediting of the actual flown miles (minimum 500) for each regular flight with Czech Airlines and airline partners.
• *OK Plus silver card / Sky Team elite*
  Benefits provided on most flights within Sky Team
• elite bonus 25%
• priority check-in
• priority airport standby
• priority boarding
• reservation waitlist priority
• preferred seating

BeneBenefits provided on Czech Airlines routes:

• possibility of upgrade to the business class / 2000 miles will be deduced from a holder's account for being upgrade on long haul flts/
• special excess baggage 10 KGS
• free admission to the abc lounge at airport Prague

Limit necessary to obtain the card:

• 25,000 miles (basic miles and business bonuses) or 15 flt segments (flt with Czech Airlines and alliance partners only).
• period to obtain the limit is one calendar year.
• the silver card will be automatically issued and sent to the holder's address *OK Plus gold card / Sky Team elite plus*

Benefits provided on most flights within Sky Team:

• priority check-in
• priority airport standby
• priority boarding
• reservation waitlist priority
• preferred seating
• guaranteed reservation

• 50,000 miles (basic miles and business bonuses) or 30 flt segments (flts with Czech Airlines and alliance partners only).
• period to obtain the limit is one calendar year.
• the gold card will be automatically issued and sent to the holder's address

Mileage upgrade

• effective January 1, 2004, OK Plus members may use their miles for confirmed upgrades of revenue tickets sold in rbd y or m in any zone on regular ok flights operated by Czech Airlines.
• passengers may also request upgrade for persons they select.
• upgrade may be requested without any limitations throughout the course of ones membership, up to the balance on the account.
• upgrade may be requested 5 days before the departure date at the latest.
• only miles from a single account may be used for the upgrade of one tkt. Adding miles together from several accounts is not possible.
• mileage upgrades are made exclusively by Czech Airlines offices on mcos that must Be issued only upon authorization from the OK Plus service centre.

The change of reservation is made to a special rbd "i"
The change of reservation may be exclusively made by Czech Airlines offices after receiving auht to issue an mco from the OK Plus service centre.

Earn miles
Each use of Czech Airlines, or other OK Plus programme contractual partners services is awarded by a respective number of miles.
There is no expiration date for collected miles.collected miles are valid for all the time of membership of OK Plus programme
Miles for actually flown flights and used services may be added only when the OK Plus programme membership is active.
For members with date of joining after 1 April 03 is possible to credit miles for Czech Airlines flts only 3 months before the date of joining. /if they present flight tkt copies and originals of boarding passes/

Each claim regarding unadded miles for (each) flight has to be accompanied by related documents, i.e., the airline ticket or its legible photocopy and the original of the boarding pass - within 12 months, at the latest. the claim has to be delivered to the OK Plus service centre
