Ok Plus application form
Application form available:
Each private individual aged 2 years and older may become a member of the OK Plus programme, if he/she complies all membership conditions.
Membership conditions are possible to find in application form or at www.ok-plus.com
New members may add their flights /Czech Airlines flights only/ flown within 3 months. Claim has to be made till 12 months from the beginning of journey.
Before the date of joining /required documents are copy of the air ticket or itinerary and originals of boarding passes/.
Ok Plus cards
*temporary card*
Temporary card is a part of application form. The passenger detaches the temporary card and fill in his/her name. A temporary card may also be printed on-line, through the application for on the Internet.
*identification card*
OK Plus members will obtain an OK Plus identification card crediting of the actual flown miles (minimum 500 miles per domestic and 1,000 miles per international flight) for each regular flight with Czech Airlines and airline partners.
*OK Plus Silver card / Sky Team Elite*

Limit necessary to obtain the card:
* If operating conditions allows and in case of ticket purchased in respective booking classes. 4,000 miles will be deducted for upgrade on flights to/from North America and 2,000 miles for upgrade flights to/from the Middle East for each leg of journey.

Spend miles
Bonus tickets
If you decide to choose a bonus ticket, contact any CSA reservations office. Because it is not possible to issue your ticket immediately, we recommend that you first contact the office by phone. A bonus ticket can be used for flights with CSA, Aeroflot, Aeromexico, Air France, Alitalia, Continental, Delta Air Lines, KLM, Korean Air, Northwest, Air Europa, Air Malta and Kenya Airways. The issuing of One-way bonus ticket is possible for OK-coded flights operated by Czech Airlines only.
Mileage upgrade
OK Plus members may use their miles for confirmed upgrades of tickets sold in price package Economy Premium (booking classes Y or M) in any zone on regular OK flights operated by Czech Airlines.
Upgrade may be requested 5 days before the departure date at the latest.
Mileage upgrades make exclusively Czech Airlines offices.

Earn miles
Each use of Czech Airlines, or other OK Plus programme contractual partners services is awarded by a respective number of miles.
Effective from July 1st, 2007 is possible to buy miles in specified amounts. More information and conditions are given at www.ok-plus.com
Member gets member. 2,000 miles will be credited to your OK Plus account for each new active OK Plus member you have recommended. For more information and conditions see www.ok-plus.com
Notice:
There is no expiration date for collected miles. Collected miles are valid for all the time of membership of OK Plus programme.
Miles for actually flown flights and used services may be added only when the OK Plus programme membership is active.
For new members is possible to credit miles for Czech Airlines flights only 3 months before the date of joining. /if they present flight ticket copies or itinerary and originals of boarding passes till 12 months from the beginning of journey/
Each claim regarding unadded miles for (each) flight has to be accompanied by related documents, i.e., the airline ticket or its legible photocopy or itinerary and the original of the boarding pass - within 12 months, at the latest. The claim has to be delivered to the OK Plus Service Centre.
It is possible to made the claim on the Internet for Czech Airlines only (OK flight, ticket with the carries code 064)
